THIS IS THE FINAL EQUIPMENT PICKUP! After this date there will be a $10 fee assessed and special arrangements will have to be made to pickup your atheletes equipment.

WHAT TO BRING:
1. All registration fees must be paid in order to pick up equipment. Please login to your account at www.sportssignup.com/lyf.start to print out a receipt and bring it with you to the equipment pickup as proof of payment. Or plan to bring your remaining balance with you (we will only be able to take cash or check at the equipment pickup if you need to pay with a credit or debit card do it online prior to picking up the equipment.) If you are experiencing financial hardship please email info@lehiyouthfootball.com to make special arrangements.
2. The Equipment Agreement Form. Click here to download, print, and sign this form to bring with you.
3. An equipment deposit check made out to Lehi Youth Football for $150 that will not be cashed unless you do not return the equipment at the end of the Season.
